Everything your doctor, the hospital and your family need to know. In one binder.

A nurse-designed health binder for doctor visits, hospital stays and everything in between.

In 23 years of nursing, I watched the same moment play out again and again. Someone arrives at the ER, and the questions start. What do you take? Any allergies? Who's your doctor? When was your last surgery? And a frightened family member says, "I'm not sure."

It's never because they don't care. It's because nobody ever told them what to write down, what to ask, or what to check before going home.

The people who did best always had someone who knew them, kept track, and asked questions. This binder helps you be that person, for yourself or for someone you love.

What's inside

Five sections, printable and fillable:

• The Essentials: your health snapshot, medications, allergies, contacts, a wallet-size Health Passport, and Medical ID on your phone

• Doctor Visit Toolkit: walk in prepared and leave with the plan in writing

• Hospital Stay Toolkit: the go-bag, "My Normal", and a page for each day

• Coming Home Toolkit: discharge questions, home care, the first 72 hours, and who to call

• When Family Steps In: one spokesperson, who speaks for you, and your wishes

Fill it in your way

• Print it and write by hand. Big print, plenty of room.

• Type into it on your phone, tablet or computer.

• Or answer the questions once online and download your binder already filled in. Your answers stay private on your own device.

The 7-Day Health Ready Plan

About 30 minutes a day for one week. Start with Section 1, skip anything that doesn't apply, and by day seven the important things are written down and your family knows where to find them.

About Rupy

I'm Rupy, a retired registered nurse with 23 years in nursing: hospital wards including the cardiac surgery ICU, long-term care and retirement homes, the community, and as a nurse educator. Much of my work was helping people move from hospital to home and setting up the care and support that kept them there. This binder is everything I wished every family had with them.

Common questions

Do I need special software? No. Print it, or use the free Adobe Acrobat Reader app, or the online form.

Is my information private? Yes. The PDF lives on your own device. The online form saves only in your own browser and sends nothing to anyone.

What's the difference between the binder and the Phone Edition? Same answers, two sizes. The full binder holds everything, for printing or typing. The Phone Edition is the key pages, sized to read on your phone without zooming, so they're with you at every appointment and in an emergency.

Can I fill it in for my mom or dad? Yes. Every page works whether you're filling it in for yourself or for someone you love. The online form keeps each person's answers separate, so you can do one for each parent.

I'm not in the US. Will it work? It's written for the US healthcare system, but most pages work anywhere. Some terms (Medicare, 911) are US-specific.

What if it isn't for me? Email hello@nurserupy.com within 14 days and tell me what didn't work, and I'll refund you in full.
